In order for Twin Enviro Services to decide whether we can lawfully and safely accept your waste for delivery, we must obtain this information about your waste. The form should be filled out completely by someone knowledgeable about the waste and hazardous waste rules. The Generator makes a hazardous waste determination and is responsible for its waste from cradle to grave. All related analyses must be included with this form, and if future analyses differ from what was submitted, the analytical results must be sent to Twin Enviro Services immediately. All information on this form must be typed or printed in black ink. This form is required for Asbestos Disposal.

This form should be filled out to be part of the documentation included with the shipment of non-hazardous waste (Petroleum Contaminated Soil or Asbestos Containing Materials, for example). The Manifest is signed by the Waste Generator and shows the waste has been relinquished to the Transporter and finally the Disposal Facility, and is used as proof that the waste was delivered to a site that is approved to accept it. A four part form is available from any of our offices, with much of the information filled out in advance for multiple load projects.
